RAPID REHOUSING

VMC’s Rapid Rehousing Program (RRH) is a housing case management program that offers assistance to adults experiencing homelessness.  Neighbors meet with their assigned case manager to develop a plan to move into permanent housing in the community.  Once housing is achieved, case managers will continue to offer support by way of home visits and connection to any needed long-term supports.  Case management is provided for a minimum of 3 months and up to 9 months post-housing placement.

Who we serve:

Knox County TN residents who are literally homeless (staying in emergency
shelter or a place not meant for habitation) who score in the range for Rapid
Rehousing on the CHAMP assessment.


What we provide:

Case management to help locate permanent, affordable housing


Cost:

Services are free


Times of service:

Monday-Thursday 8:00 am – 5:00 pm; Friday 8:00 am – 3:00 pm


How to get services:

We offer services to those who have been referred by the Coordinated Housing
Assessment and Match Plan (CHAMP) assessment and have completed an
orientation session. Those interested in RRH should start with a CHAMP
assessment that can be completed in the VMC Bush Family Refuge
Tuesday/Wednesday/Thursday 9:00-11:00 am.


The Rapid Rehousing waiting list is reviewed on Thursdays. Individuals are
selected from the waiting list based on CHAMP score and case management
capacity. In order to be selected for the program, neighbors must maintain
contact. Referred neighbors may check in during in person office hours
(Thursday 3:00-4:00pm) or by responding to phone calls or email from RRH
staff.


What to bring:

No documents are required; staff will confirm the CHAMP referral via HMIS
